The information below represents a non-exhaustive list of FCC and state regulatory assessment and reporting requirements coming due September 1 through November 1 including the quarterly FCC Form 499Q due November 1, 2005.  The filing deadlines are generally applicable to interexchange or long distance telecommunications service providers, including switched and switchless resellers as well as prepaid providers.  If your company holds an FCC 214 authorization or provides intrastate toll service in any one of the states listed below, chances are that one or more of the requirements covered in this Alert may apply to your company.  Interest on late payments and/or other penalties, including loss of state certification, may apply if filings or payments are not submitted on time.

FCC Form 499Q- Due November 1st.

 

Alaska

Quarterly RCC Report- Due no later than October  30th.

 

California

Combined California PUC Telephone Surcharge Transmittal Form - Due monthly.

PUC User Fee- Due October 15th.

 

Indiana

Public Utility Fee- Due October 1st.

 

Nevada

 Quarterly Assessment- Due October 1st.
